Preparing for your first appointment

If you have seen another doctor and/or have received treatment before for the same condition you are coming to our facility for, please call the front desk of that doctor/facility and ask they fax your records to 877-813-6315. If you have had an MRI, x-rays or other images please bring them with you to your appointment.  If you need help with this please call our front desk at 567-940-9334.

Please bring a list of your medications including: prescriber, reason for taking and dosage. Alternatively You may bring the bottles with you.

New patients must make a $100 deposit for their initial appointment. Deposit shall be credited toward cost of examination, x-rays and or any services rendered at our facility. Deposit non-refundable if appointment is not cancelled or rescheduled 48 hours in advance or in the case of a no show. If your appointment is on a monday you must cancel by friday at 5pm.

To cancel please call 567-940-9334. If you are cancelling and for whatever reason reach our voicemail please leave a detailed message for us to return your call. 

If you would like to complete your intake forms prior to your appointment please call our front desk at 567-940-9334 and request them to be e-mailed or mailed via USPS.

We prefer to only have the patient in the examination and x-ray rooms unless you have a condition that requires you to be accompanied or you request that you are accompanied. 

Children are welcome in our facility and we treat many! To protect them we do not allow them to be unattended. Please keep in mind that the initial examination may include an x-ray examination which will require any adult or children other than the patient to leave the room.  

We will perform a thorough medical exam, orthopedic tests, hormone evaluation and x-rays if needed. If additional labs, imaging or medical records are needed we will arrange them. Depending on your condition, how long it takes for you to complete new patient intake forms and how long you discuss your condition with our providers your appointment will take anywhere from 60-90 minutes.

We will perform a report of findings and encourage you to bring someone that helps you make healthcare and financial decisions. We will go over the results of our examination and make recommendations based up on your condition. There is no cost for this appointment and depending on your questions the appointment can last anywhere between 20-40 minutes.

If for any reason you have a question after your report of findings you may call and schedule another appointment / a phone call to have those questions answered. 

Phone call: $30 for first 15 minutes and an additional $20 for every 15 minutes thereafter.

Second Report of findings: $50.
